Introducing a new role for BNP: as a general indicator of cardiac structural disease rather than a specific indicator of systolic dysfunction only.

چکیده
In the early 1990s, the idea first arose that plasma concentrations of B type natriuretic peptide (BNP) might help target scarce cardiac investigations towards the most deserving. This mainly referred to the possibility that BNP could detect which patients had heart failure and which did not. This was based on the fact that echocardiography slots were limited, and that heart failure was hard to diagnose accurately on clinical grounds. The need for correct targeting of echocardiography increased when it was fully realised that clinical heart failure was a heterogeneous entity and that the treatments which produced major benefit (angiotensin converting enzyme (ACE) inhibitors and β blockers) were only of definite benefit in those with left ventricular (LV) systolic dysfunction. A huge amount of work then went in to establishing how well BNP identified either heart failure or LV systolic dysfunction. 4 The bottom line of this work is that a normal BNP is a very effective way of “ruling out” heart failure or LV systolic dysfunction in either a symptomatic or an asymptomatic patient. In this way, echocardiography could be reserved only for those with a high BNP concentration. However, it became patently clear that a high BNP in itself did not always indicate LV systolic dysfunction. Thus for diagnostic purposes BNP was sensitive but not specific—that is, although a high BNP was picking up nearly all LV systolic dysfunction, it appeared to also pick up some other patients. This begs the obvious question: what other patients were being identified by a high BNP concentration?

Echocardiographic Epicardial Adipose Tissue as a New Indicator of Cardiovascular Risk
Abstract Background and Purpose: Epicardial fat is the true visceral fat located around the heart, particularly around sub-epicardial coronary arteries that may locally interact and modulate the coronary arteries and myocardium through paracrine or vasocrine secretion of anti-inflammatory and proatherogenic cytokines. Most previous studies have used echocardiography and reported controversia...
متن کاملClinical and Biochemical Assessments of Circulating B-Type Natriuretic Peptide as a Useful Marker in Pediatric Cardiac Patients
Background Although the left ventricle is the major site of BNP secretion in response to cardiac pressure or volume overload, the myocytes of both atria and ventricles secrete B- type natriuretic peptide (BNP). This study aimed to assess and compare the plasma levels of BNP in common pediatric cardiac diseases to clarify its pathophysiological role and evaluate its po...

Background: Cardiac dysfunction is one of the major causes of morbidity and mortality in thalassemia patients. This study aimed to compare the effect of Brain Natriuretic Peptides (BNP) and Calcitonin Gene-Related Peptide (CGRP) with echocardiographic findings in early diagnosis of cardiac disease in major thalassemia patients. Materials and Methods: This study was performed on 80 patients amo...
متن کاملN-Terminal ProBNP: Marker of Systolic Dysfunction or Nonspecific Indicator of Cardiac Disease?
Accessible online at: www.karger.com/hed The clinical diagnosis of heart failure may represent a considerable challenge, particularly in the obese, in women and in the elderly [1]. Because contemporary heart failure therapy has been shown to significantly reduce mortality and the number of hospital readmissions, an early and correct diagnosis is crucial. The cost and potential side effects asso...
